FamilyCare Hospice
FamilyCare Hospice provides care that helps you achieve the highest quality of life possible during a terminal illness. The hospice philosophy includes care for patients and their families to relieve or deal with physical, emotional and spiritual discomfort. A personalized care plan is designed for each patient and allows the patient to stay in his or her own home. Hospice Medical Social Workers provide a listening ear and help guide families with emotional support during a terminal illness.
Hospice Services may include:
- Symptom and pain control.
- Home care visits by hospice-trained nurses, aides, and other caregivers.
- Chaplain available for spiritual support of patient and family members.
- Hospice Medical Social Workers for emotional support and information.
- Bereavement support, which continues for family members for at least one year after death.
Hospice Eligibility
- Patient is diagnosed with a terminal condition.
- Patient has a life expectancy of six months or less as measured by his or her physician (though hospice services may continue even after six months).
- Patient has physician approval of the hospice plan of care.
- Patient accepts Hospice philosophy and goals.
- Patient lives in Culpeper, Madison, Orange, Rappahannock or southern Fauquier.
