FamilyCare Home Health

CRH FamilyCare Home Health is proud to be the only home health program to receive the AVATAR International Five Star Award for exceeding patient expectations in 2005.

When you are recovering from a serious illness or injury and want a more comfortable alternative to an extended hospital stay or nursing home care, FamilyCare can assist families and homebound patients with personalized healthcare in your own home.

Each patient is assigned a team of caregivers that is directed by the patient's own physician. Depending on individual needs, this could include nurses, aides, physical, speech and/or occupational therapists, and/or a medical social worker. The members of your team will provide prescribed medical care, education and motivation to assist you in your recovery.

Home Care is usually covered by private insurance. FamilyCare is also certified for Medicare and Medicaid patients. FamilyCare Home Health and Hospice is accredited by the Joint Commission on Accreditation of Healthcare Organizations.

Home Health Services may consist of:

• Skilled nursing care in your home, which may include teaching you and/or family members care techniques allowing you to stay at home.
• Home health aides certified to provide personal care, grooming, even meal preparation when necessary.
• Physical, occupational and/or speech therapists based on your care needs.
• Pain management assessment, if needed.

Home Health Eligibility

• Patient is under the care of a physician.
• Patient needs skilled nursing care and other therapies.
• Patient has medical or family care needs which can be met at home.
• Patient is homebound and needs the assistance of others or equipment to leave home.
• Patient lives in Culpeper, Madison, Orange, Rappahannock or southern Fauquier.
